Tell him to eat more fat instead of nuts, which tend to be carby. If he tolerates dairy, try lots of heavy cream in the coffee. Very dark chocolate can also help. As far as not going to a restaurant, the filler (bread, tortilla chips, rice, etc.)... (more)
