<p>So after sustaining what is possibly <a href="http://paleohacks.com/questions/166712/hack-my-titanium-implants#axzz2Fo6BPWGM" rel="nofollow">the least Paleo injury ever</a> (I tripped over my Italian hand luggage) I have been left with what the UK top shoulder consultant described as "the unholy trinity" of shoulder injuries: dislocation, fracture and nerve damage. </p>
<p>Basically (during the relocation) a large section of my hunerus sheared off (from the ball joint down) and is now held in place by a fine piece of German titanium engineering. Such is the size of the fracture, my consultant has warned me of the significant risk of "osteonecrosis of the humeral head" which means that the reattached portion of the bone will not rejoin, and will "die", leaving me with arthritis and the prospect of a shoulder replacement in my early 40s.</p>
<p>So hack me this: What dietary (or supplemental) adjustments can I make to give me the best possible chance of healing my broken bones?</p>
<p>I've never tried it before, but I think that bone broth is probably a no-brainer (surely the best way to get lots of bone building minerals).</p>
<p>But what about low-carb and IF. I've recently been following a <25g a day leangains type thing (to control my T2 diabetes). Is there any truth that low-carb diets are non-ideal for bone health? I' m scared that keeping myself in ketosis (which is essentially a starvation state) is not going to encourage my body to repair itself...</p>

<p>Is it because we evolved to eat things that didn't bite back? I can believe that is partially true, but there must have been opportunities to wing the occaisional carnivore and take it home for supper.</p>
<p>Is it a neolithic remnant? It must be easier to farm herbivores than livestock that were liable to eat each other or require a constant supply of live food.</p>
<p>Or is it a biomagnification thing? But then aren't things like mercury pollution modern concerns? Would they have been such a big problem for our ancestors?</p>
<p>I'm not sure if it's apocryphal, but won't eating the livers of carnivores (such as polar bears or alligators) give you Vitamin A poisining? That seems to be a clear indication that we haven't evolved to eat them.</p>
<p>What would happen if I completely replaced the ruminants in my diet with the meat from carnivores? Would it have a deleterious affect on my health?</p>

<p>Once you've got your BMR you need to adjust it based on your activity levels:</p>
<ul>
<li><strong>sedentary</strong> (little or no exercise) : Calorie-Calculation = BMR x 1.2</li>
<li><strong>lightly active</strong> (light exercise/sports 1-3 days/week) : Calorie-Calculation = BMR x 1.375</li>
<li><strong>moderatetely active</strong> (moderate exercise/sports 3-5 days/week) : Calorie-Calculation = BMR x 1.55</li>
<li><strong>very active</strong> (hard

<p>So even the little dude we described above would be burning ~2500kcal a day lugging boxes all day. If you are a big strapping lad, I'm sure you'll burn much more.</p>
<p>Obviously these are only estimates (although calorimeter studies show that they vary little from person to person), but they show you that 1400kcal is completely unsustainable (unless you are extremely obese).</p>

<p>I'd take a look at Paul Jaminet's book - in which he does some calcs to show that the maximum amount of carbohydrate that your liver can process is about 150-160g (unless you are some sort of elite athlete).</p>
<p>Going above that amount isn't necessarily a bad thing, but your metabolism will have to start relying on secondary disposal pathways. The least harmful of these is <i>de novo lipogenesis</i> in which your liver will convert the excess carb to saturated fat. Jaminet argues, that if you're better off eating fat directly, than converting carbohydrates to fat. This is because the lipogenesis process is very slow, and causes hyperglycemia as the carbs are slowly converted to fat.</p>
<p>Jaminet, Harris, and Sissons all come up with different arguments for about 150g a day of carbohydrate as a reasonable maximum. In my own work with T2 diabetics I've also seen that 150g is about the maximum any diabetic can eat without suffering extreme hyperglycemia (most diabetics can eat far less).</p>

<p>They just don't understand. The're confusing ketosis with ketoacidosis. The two things are completely unrelated.</p>

<p>The HbA1c numbers are great, but they are really a function of my Very Low Carb diet (<25g a day), and don't necessarily reflect a change in insulin resistance. </p>
<p>In order to give me a yearly benchmark, I decided to perform a DIY Oral Glucose Tolerance Test (OGTT) every year on January 1st 2012. <a href="http://www.eattoyourmeter.org/blog/diabetes-mot-the-diy-oral-glucose-tolerance-test/" rel="nofollow">This year's test said that I was "pre-diabetic"</a>, unfortunately I don't have a comparison for diagnosis, but I hope to should a constant improvement year by year (as my weight reduces further) to the extent that I am no longer a diabetic.</p>

<p>His point is that the First Law tells you HOW fat accumulates, it doesn't tell you WHY. Taubes thinks that the answer to the "WHY" question is mainly down to hormonal reaction to carbohydrate.</p>
<p>I wish we could just get past the whole "a calorie is not a calorie" thing. I highly recommend either of Taubes books (even if you don't agree with his carbohydrate hypothesis).</p>
