MOAR BACON. Seriously pound some bacon, wings, and ribs. Any fatty, dense meats. If you really can't handle eating food, start making shakes. Protein powder, butter, coconut milk, etc.

Lactose may not be the problem. Lots of people cannot tolerate casein, which is a protein found in milk. There is no pill to reduce casein, like there is for lactose. A big dose of fat on an empty stomach may cause reflux.

For me, starting off with liquid coconut oil on an empty stomach usually = upchuck city. A lot of people need to start off small and build up with it.

The only carbs you get are from an apple and a banana, try a potato or a sweet potato and see if that helps. There's no reason to be doing low carb at your weight.
