I can't help you with any chemistry but maybe try doing something active to stimulate your appetite. I know when I don't eat and go work out early in the morning, I get very hungry very soon after starting, even if I had no appetite before. (more)

Lactose may not be the problem. Lots of people cannot tolerate casein, which is a protein found in milk. There is no pill to reduce casein, like there is for lactose. A big dose of fat on an empty stomach may cause reflux.

For me, starting off with liquid coconut oil on an empty stomach usually = upchuck city. A lot of people need to start off small and build up with it.

The only carbs you get are from an apple and a banana, try a potato or a sweet potato and see if that helps. There's no reason to be doing low carb at your weight.
