I do weights sessions 3 times a week, doing the 5x5 stronglifts routine. I eat 3 meals a day, which goes roughly like this - low carb/mod protein/high fat. My post workout meal is usually either my lunch or dinner. Is this ok or should I add in a 4th meal?
I'm 250 lbs, just under 6foot, and my goal is fat loss/build muscle mass.

I presume your "main" goal at the moment is weight loss? How are you getting on with your current setup?
I would stick with 3 daily meals for now, then when you approach your target weight and wish to add some muscle, look at adding an extra meal post workout or simply added some paleo friendly carbs to the post workout meal (sweet potatoes, squash, parsnips etc).
