Yes. I've lost (at least 70lbs) on <25g carb and have leg muscles like tree trunks.

Yes, the combo of Paleo and crossfit totally changed my body composition. The exercise was important though.

Ditto - PHD for 1.5 years. I lost lots of belly fat (internal) and could see my abs somewhat but only when I started biking to work every day (360kcal/day) I suddenly had an even flatter tummy and better defined abs. – wmertens Oct 12 at 13:05

Certainly. I have lost 20lbs of fat & at least doubled my strength in all the things I measure. No idea how much muscle this equates to. But you can see it for the first time.

I lost 30lbs. As I was losing I could swear that I was gaining muscle, too. I could have simply been revealing it, however. I do feel stronger, more vital, more energetic, more durable, so I feel I can answer Yes to your question.

Yes. Definitely lost fat (around 16lbs, and I wasn't exactly overweight before) and gained lovely muscle definition.

One caveat: the above has been true everywhere but the stomach area so far--I still have a lil' layer that doesn't seem to want to shift. As I'm now slim and toned everywhere else, I figure that's how I'm supposed to be, give or take, and I'm happy with that. :)
