Not sure about your situation specifically, but I've been sorting out some similar issues in my shoulder...and I found the solution by hunting paleo (info, not animals). Robb Wolf's podcast led me to Tim Ferriss' blog, which led me to his book, which took me to an amazing guy named Gray Cook, which landed me serendipitously to a student of his in my home town in Nova Scotia.
Essentially, the common link above was something called FMS - Functional Movement Screen. A trained and certified practitioner can assess and diagnose biomechanical imbalances and injuries that lead to things like shoulder and elbow problems. I've been to a ton of physios, and no one has EVER found the things that this guy did.
Solutions have included ART (Active Release Technique), targeted mobility and strength exercises, and ergonomic adaptations (standing desk, better shoes...). It's been amazing.
