I wouldn't be concerned about the starch. You need calories right now and you should eat until you're not hungry anymore and don't feel bad about it. I think sometimes women feel guilty for having a large appetite. I'm breastfeeding my 6-month-old right now and I eat a ton. We have/had some of the same restrictions you do so I might have some ideas for you. I do eat rice though. Dinner I usually have meat + starch + veggie + fat and lunch is usually dinner leftovers. Breakfast is always coffee w/coconut milk in it plus something starchy and fatty (sometimes proteiny, sometimes not). So what does that look like in real life? I'll try to remember some stuff I've eaten lately...they're not in order or the leftovers would match up. :)
DAY 1
breakfast: coffee + coconut milk, sweet potato hashbrowns cooked in bacon grease, sausage
lunch: leftover roasted pork shoulder and roasted carrots (the pork had a thick layer of fat on it, carrots were roasted in avocado oil)
snack: a banana and an avocado mashed together
dinner: chicken fesenjan http://www.whats4eats.com/poultry/fesenjan-recipe with saffron rice pilaf and cabbage salad (you can use any fat for the chicken, and i also made my rice with lots of fat and homemade chicken stock, and the salad with extra virgin olive oil)
dessert: homemade strawberry ice cream made with coconut milk, eggs, strawberries, lemon juice, and a very small amount of honey
DAY 2
breakfast: coffee + coconut milk, tapioca pudding made with coconut milk, shredded coconut, and mashed taro root
lunch: leftover cajun shrimp (previously cooked in bacon grease) and dirty rice made with bacon + grease, ground beef, and beef liver along with assorted flavoring veggies
snack: a banana and macadamia nuts
dinner: stir fry of beef sirloin, cabbage, carrots, and onions done in coconut oil and a few asian seasonings (sesame seeds, scallions, etc)
dessert: dark chocolate
DAY 3
breakfast: coffee + coconut milk, 2 poached eggs, an apple
lunch: leftovers of flank steak with chimichurri sauce, plantains cooked in coconut oil, and cabbage salad w/evoo dressing
snack: tapioca pudding made with coconut milk, shredded coconut, and mashed taro root
dinner: roasted chicken, mustard greens braised in chicken stock and coconut oil, rice cooked in coconut milk, and mango salsa
dessert: homemade chocolate ice cream made w/coconut milk
