I've been paleo for a year with about a month's worth of indiscretion over the holidays and still lost a considerable amount of weight, resolved the majority of my stomach issues, as well my seasonal environmental allergies (hay fever, etc...).

That being said, I'm recently having a resurgence of stomach problems, while otherwise feeling great. Very foul smelling, very loose stool... considerable urgency upon onset... I've noticed these considerably increase when I eat starchy food, i.e. potatoes or plantains. If I'm zero carb, the occurrence is very rare... so lately it's been no carbohydrate at all.

While not "painful" like movements were in my gluten days (like passing lava), these are still pretty uncomfortable.

A good friend of mine that went on a trip with me (pre-paleo days) commented that my frequent stomach upset and gas were signs of h.pylori and I should get checked out.

I am wondering, if gluten is my issue, or if I have a long-standing parasite that is thriving on the starch that I'm not fully digesting?

Regardless of answers - I still plan on getting tested. It's better to know, but perhaps my experiences will help fellow Paleohackers...

I stayed zero carb, with occasional carbup days lasting less than 10 hours... probiotics and fermented food (kimchi) the day after. – Joshua Jun 14 2011 at 0:16

If you have checks in all 3 sections, you and your doctors should definitely explore a celiac diagnosis.

sorry to hear that. after eating paleo for a year and only 1 month of cheating, your blood tests are not likely to be accurate. That may not matter to you, but could be important for family members. Are you well versed in your family medical history - if there are autoimmune conditions, that could be a clue. – Egk May 2 2011 at 2:12
I had almost all of these symptoms, and was tested for celiac, wheat allergy, etc all negative. do you have any reason to suspect candida overgrowth, Especially repeated courses of antibiotics? Conversely, if you suspect h. Pylori, a positive test would require antibiotic treatment. H pylori is indicated in lmot all stomach cancer, and the rate of stomach cancer in indiv. With h pylori is quite high. My FIL was as close as you got to Paleo in the 70's and 80' s - big hunter/ fisherman, lived in Alaska for years, ate game, wild-caught salmon, few carbs.....died of stomach cancer at 52. You sound like you have SIBO -- overgrowth of microbes (yeasts, bacteria) in places they don't belong -- the small intestines which is nearly sterile in asymptomatic and healthy individuals.

Consider trying the (elimination) diet that works for you (no starch, lactose, fruit/high-fructose-corn syrup (berries might be ok), sugar alcohols, inulin/FOS or sweets/sugar) for 2-4 wks then gradually re-introduce 1 food at a time. This is a common reason for 'fail-eo' on paleo despite going gluten-free -- I had this too when I hit the croissants in France last year! I was set back by 1-2 mos of bloating so last time for big time non-paleo-goodies FOR ME. Interestingly monosaccharides are ok (like pure glucose and ribose) because they do not require pancreatic enzymes or enzymes from our damaged villi/brush border/GI lining.

What helped me too was

--allibiotic (garlic and coconut oil/caprylic acid are anti-microbial) from NOW foods (on iherb.com)

--super enzymes again from NOW

--working out (this affects GI motility and I think brief hypoxia to the pathogenic organisms)

--probiotics (sometimes this makes SIBO worse -- b/c you are adding to the fire-- but in most cases adding lactobacillus in IBS/SIBO studies will 'cure' the symptoms and breath tests a great majority of the time)

If there is leaky gut/permeability issues, over time it should improve but many things heal this faster (vitamin D repletion, NAC, aloe, lactobacillus, S.boulardii, glutamine, high quality $$$$ omega-3, butyrate/ghee, etc).

Don't worry about h.pylori yet. The microbes and yeasts are bigger targets. Parasites are actually bigger public health threats than people think... I wash all my veggies and fruit now in a 10-20 min lemon juice or vinegar bath (dilute).

I'm in a similar situation and I have found that testing for gluten allergies doesn't always come up positive even if you know you are reacting to them. Caseins are a possibility as well.

I'm doing alot of reading on GAPS, it suggests that sometimes you wont test positive for things until you've done a considerable amount of damage. Also try reading up on leaky gut and see if anything resonates there.

Are you taking any kinds of probiotics? according to GAPS (I am currently doing this) you can work up to a therapeutic level of probiotics to fight off the bad bacterias you have and get the good ones in control. If you are like me, I've been sick for years with stomach issues so my stomach acids are gone as well as good bacteria. It can take up to 4 months to get up to a full probiotic dose (suggests 15-20 billion) and you need to take that dose for 6 months. Supposedly this will get rid of any parasites and yeast issues as long as you are also controlling your diet. I'm still building up my dose but if done slowly you wont suffer too much die off effects (as the battle begins in there), and I've already noticed a change. (theres also stuff about fermented foods like homemade sauerkraut that is helpful too)

I had really wretched problems with my stomach for YEARS. Gluten free helped (have been working with Zero Carb/Low Carb/Paleo since 2008) but I found my stomach problems resurfaced again about a year later ... when I started taking supplements again.

For me, I found out it was a sensitivity to some of the newer, artificial sweeteners in my chewable vitamins or cold meds. I'd kicked the Diet Coke habit, and no longer ate any sugar-free, low carb treats -- but had never thought to look at what was in my supplements. Far too many have artificial sweeteners.

Been fine since. Hope you feel better soon!

I have had these problems for year too - paleo has not helped me and I would say even made things worse, until I added back potatoes a few times a week. Mornings are still bad - esp after breakfast (doesn't matter what the food is) but it generally settles down after lunch.

I've started not having any supplements until after lunch now - when even magnesium citrate seems to be tolerated!

I'm eating masses of home made sauerkraut in the hopes that will restore order - but so far it is a vain hope. And I love veg too much to give it up. Plus, I have this inbuilt belief that vegetables do confer all sorts of miraculous benefits which I need.

If you find a cure, or something that brings your intestines peace, do let me know - it is an answer I have been looking for for around 40 years!!!

Funny that the taters are what caused mine to flareup! I would say talk to your doctor and get a stool panel done, I've actually been fine since going absolutely zero carb+extremely high fat for the past three days (carnivore + heavy cream + fish oil + coconut oil). I still plan on getting the stool panel done, so I know for sure and can combat it. – Joshua May 3 2011 at 1:02

These symptoms could be H pylori, but more likely to be related to a parasite like Giardia or Blastocystis.
