Does anyone know what would cause HIGH serum testosterone and low (.2) free testosterone

High SHBG may do that.

Are you taking oral contraceptives or acne medication?

How is your blood sugar? High insulin can also affect it.

Hyperthyroid can also cause that.

i am thin with high blood fasting blood sugar and a LOT of stress. i am not hyperthyroid..if anything hypothyroid at the time i tested this my tsh was 4.something..higher than it's ever been. recently i went VLC and im thinking i went a little overboard. a few months ago (not vlc) my testosterone (total) was normal..unfortunately i didnt test free. hrmm – holly Jan 2 2012 at 16:11

I do not know a whole lot about this topic, but there seems to be some connection there. Do you have any autoimmune disease? Have you been gluten free? I ask because that could have an effect on your pancreatic B-cells which make insulin. In contrast, you could have some insulin resistance going on. VLC is a stressor and if you have a lot of outside stress I'd consider having ~150g carbs/day to avoid the self-induced low carb stress – Jeff Jan 2 2012 at 17:13
